Management of Steroid Induced Diabetes with Unani Medicine: A Case Report

Background: Glucocorticoids (GC), such as prednisolone, are commonly used for their anti-inflammatory and immunosuppressive properties. However, chronic GC use can lead to adverse effects, including diabetes. This case study evaluates the efficacy of Arq Afsanteen, an Unani compound drug, in managing GC-induced diabetes. A 50-year-old man with a family history of type 2 diabetes and a history of partial gastrectomy was diagnosed with sarcoid tuberculosis and began treatment with ATT and prednisolone. Four months prior, his HbA1c was 6.1%, but it increased to 7.9% after prednisolone treatment. Physical examination revealed a slightly cushingoid appearance. Laboratory findings indicated GC-induced diabetes. Intervention: Glycaemic control was improved for the patient when a 1600 kcal/day diet was implemented. But postprandial blood glucose levels persisted in being elevated. Arq Afsanteen was administered at 60 ml twice daily, one hour before meals, for 60 days. Results: Glycemic indexes improved significantly over the treatment period: fasting blood sugar (FBS): decreased from 145 mg/dl to 103 mg/dl- postprandial blood sugar (PPBS): reduced from 230 mg/dl to 127 mg/dl- HbA1c: decreased from 7.9% to 5.3%. Conclusion: Arq Afsanteen effectively managed GC-induced diabetes in this case, reducing the patient’s HbA1c from 7.9% to 5.3% in two months. This suggests that traditional Unani medicine may offer a viable alternative for managing steroid-induced diabetes. These results need to be confirmed by bigger cohort studies.
