Abstract
Background: Hemorrhoids appear frequently among anorectal disorders since they create vascular swelling together with pain symptoms, while leading to bleeding and mucosal tissue tending to slide out. Though effective therapy exists, traditional treatments produce various adverse effects and lead to recurrence and inconsistent patient following of medical recommendations over the long term. Plantbased therapeutic medicines have gained rising demand, which leads experts to investigate medicinal plants with established pharmacological effects. Methods: The scientific databases Scopus, PubMed, Web of Science, and ScienceDirect were thoroughly searched. The phytoconstituents, pharmacological activities, traditional uses, and formulation strategies of Terminalia arjuna were compiled as well as analyzed through considerable relevant articles. Results: Bioactive compounds found in the Terminalia arjuna bark include flavonoids, tannins, saponins, and triterpenoids, which are used against inflammation, as antioxidants, astringents, and for wound healing. It has been extensively studied for cardiovascular indications but is less known to have been utilized for proctological use. Its pharmacological profile is consistent with its rational use in hemorrhoid therapy, as an antiinflammatory, antivascular congestion, and antiproliferative agent. It also highlights the potential for such advanced delivery systems as topical nanogels and suppositories. Conclusion: Terminalia arjuna is a promising herbal candidate for hemorrhoid management because of its multimode pharmacological effects. Although it is critical to have quality-designed preclinical studies, clinical trials, and formulation optimization to translate traditional knowledge to evidence-based therapeutics.
