It is challenging to transport big biotherapeutics over the blood-brain barrier (BBB), and biologics’ clinical success in treating a variety of peripheral disorders has not translated to CNS applications. A BBB delivery strategy must be incorporated into drug development for CNS-targeting treatments because the great majority of biologics with CNS targets that have entered clinical trials have not been developed to cross the BBB, which may be a factor in their lack of efficacy. Exploiting receptors expressed by the brain endothelium, whose physiological function is to promote the transport of endogenous protein ligands from the circulation into the brain via a receptor-mediated transcytosis (RMT) route, is a promising method for delivering medicines across the BBB. RMT-related receptors can be targeted by antibodies, which can then be employed as molecular Trojan horses to deliver therapeutic payloads over the BBB.
