Abstract

The diagnosis of chronic wounds poses an interdisciplinary and interprofessional challenge. In routine clinical practice, the appropriate diagnostic workup is frequently carried out insufficiently. In addition to financial obstacles, those involved in the process often lack the required knowledge, a circumstance that must be overcome. Today, the treatment of patients with chronic wounds is thus, commonly, merely symptomatic and frequently not accompanied by determination of the exact cause. The ABCDE algorithm is a simple and systematic approach for diagnosing chronic wounds, designed to be easy to remember and apply in clinical practice. It breaks down the key factors into five categories: A – Anamnesis, which involves gathering the patient’s medical history and understanding the context of the wound; B – Bacteria, focusing on identifying any bacterial infection present in the wound; C – Clinical  examination, where the wound is visually and physically assessed; D – Defective vascular system, evaluating whether underlying vascular issues are contributing to poor healing; and E – Extras, encompassing additional factors that may affect wound healing, such as patient comorbidities. The ABCDE framework is especially useful in identifying local wound infections, guiding decisions about the use of local antimicrobial therapies, and helping clinicians monitor treatment effectiveness over time. This structured method provides a clear, evidence-based approach to documenting and managing chronic wounds, leading to more informed clinical decisions and better patient outcomes.
