Special Issue Description

Heart failure has been identified as an emerging epidemic and is one of the leading causes of death worldwide. Heart failure is a major economic and healthcare burden, with a 5-year survival rate of 50%. Despite broad interests and pressing clinical needs, our understanding of heart failure remains limited. As a result, there is no current cure. The heart is an omnivore, constantly consuming fuel and utilizing any substrate available. High rates of ATP production and turnover are essential for maintaining cardiac contractility and delivering blood and oxygen to other organs. Normal cardiac ATP is primarily derived from fatty acid oxidation (FAO), with glucose metabolism contributing less. However, under stressful conditions, FAO may be reduced, resulting in increased glucose utilization. Glucose transporters (GLUTs) mediate glucose uptake in cardiomyocytes, with GLUT1 and GLUT4 being the most abundant isoforms. The high rates of ATP production and turnover are critical in maintaining cardiac contractility to deliver blood and oxygen to the other organs. Under normal conditions, cardiac ATP is mainly derived from fatty acid (FA) oxidation (FAO), with glucose metabolism contributing less. In CHF, sympathetic system activation not only increases insulin resistance but also decreases insulin release from pancreatic beta cells, increases hepatic glucose production by stimulating both gluconeogenesis and glycogenolysis, and increases glucagon production and lipolysis.
