Abstract

n

nPolypharmacy, which refers to the practice of taking many medications at the same time, is becoming more common among elderly populations as a result of the steadily increasing prevalence of chronic conditions. The clinical and therapeutic consequences of polypharmacy are investigated in this paper, with a particular emphasis placed on drug-drug interactions, altered pharmacokinetics, and patient safety. The physiological changes that occur with advancing age have a substantial impact on the absorption, distribution, metabolism, and excretion of drugs, which in turn increases the likelihood of adverse drug reactions (ADRs) and therapeutic failure respectively. We examine the various methods that are now being utilized for the purpose of optimizing medicine, such as deprescribing protocols, clinical decision support technologies, and personalized pharmacotherapy techniques. In addition, the article covers the role that pharmacovigilance plays in the monitoring of drug safety and provides an overview of upcoming technologies, such as pharmacogenomic testing and artificial intelligence, that are aimed at enhancing the management of medicine for elderly patients. The purpose of this review is to highlight the urgent need for focused treatments to prevent medication-related harm and enhance therapeutic outcomes in aging populations. The review places an emphasis on a patient-centered, interdisciplinary approach.nn
