Heat and cold therapy in patients with musculoskeletal injury

Special Issue Description

Non-pharmacological treatment for acute musculoskeletal injuries involved reducing pain and promoting healing to facilitate a return to normal functions and activities. Heat and cold therapy modalities are commonly used to facilitate this outcome. The physiological effects of cryotherapy include pain relief, decreased blood flow, edema, inflammation, muscle spasms, and metabolic demands. There is limited evidence from randomized clinical trials (RCTs) supporting the use of cryotherapy after acute musculoskeletal injury and late-onset myalgia (DOMS). The use of heat therapy includes pain relief and increased blood flow, metabolism, and connective tissue elasticity. Topical heat or cold acts as an anti-irritant, thus, increasing the activity of unmyelinated C-fibrils by inhibiting signals in the spinal cord and brainstem. By reducing muscle temperature, the axial sensitivity is reduced, and overall muscle tone. Using heat therapy decreases gamma fiber activity in muscle, reducing the sensitivity of the extensor axis. Heat can also stimulate regions of the brain, causing psychoactive effects.
